We are lucky in the Uk that everyone has equal access to the NHS.

I see lots of developing countries where they play a lot of football, even informally and probably do more exercise than many in wealthier countries.

I think there's a mindset issue too. Based on expectations of life whether you consider yourself poor or rich is sometimes unique to the individual. I don't have a phone or car, and I walk around in scruffy clothes, but I do have things that are important to me.
