Just gotta say, as someone who tried to fill it out, and isn't involved in anything except reading and writing on Medium, it's actually impossible to do it. You require all fields to be completed, even when we've never had any experience of the things questioned. I filled out the bits I did have experience of... the publication, and the newsletter - which I don't recall subscribing too, but I got one a few days ago. But couldn't submit, until I'd completed all fields. I gave up. This may be why your responses are low.
